Instructions for Accessing the Health Risk Assessment (HRA) and Online Tools

Go to www.healthytogether.net/sju
If you are using the site for the first time, skip the User Name and Password sections.
Click on the Sign Up button.
Click on the First Name box and enter your First Name, click on the Last Name box and enter your Last Name, click on the Date of Birth box and enter your Date of Birth (example: 07/04/1979).
Please use the following for your Member ID:
- Oxford & Aetna Members: Member ID number is found on the front of your ID Card
- HIP and Empire Members: Member ID is the ID number found on your paycheck
Click on the Sign Up button, which will take you to the Profile screen.
Once you have clicked on Profile, you will be asked to enter a user name. Your user name can be whatever you choose.
Continue to fill in profile questions. You will then be asked for your email address, which is required to continue.
You must create a password, which needs to be at least 7 characters, either letters or numbers with 1 symbol (i.e. @ # $ %). Example suewoods%. You must re-enter the same password again to confirm!! Hint: Write down your User Name and Password and secure in a safe place.
Please click the Update button, which will take you to an Agreement screen.
Read the Agreement and click Continue to agree to the terms.
Click on APS/HealthMedia (written in red at the bottom of the screen).
The HRA and online modules available can be accessed by clicking them under Your Available Programs.

Don’t have access to a computer or a valid email address? You also have the option of completing the HRA through the mail.
